It seems that only the armorer is supposed to be the one that handles the guns, the armorer checks the guns, the armorer gives the guns to the actor. only. no diversion from those rules.
...and yet the assistant director picked up a gun that they seemingly didn't know was loaded, declared it as a cold gun despite it being loaded, and gave it to the actor.
His lack of sticking to gun safety protocols has been noted by others, he'd already lost a prior job because of it. There have also been anonymous reports from staff of previous movie sets that he is aggressive towards younger staff.
I've got my own thoughts on how this incident happened, it'll be pretty interesting to see what more comes out about it.
